HOW TO CLAIM A BALANCE

1. Locate, from the Table of Contents, the beginning of Part I for the first institution. Part I is an alphabetical list of creditors.

2. Locate, in consulting the alphabetical list of creditors, the name you are interested in.

3. Locate, opposite the name you are interested in, the code shown in Column 6.

4. Locate, from the Table of Contents, the beginning of Part II for the first institution. Part II is a list of branch addresses.

5. Locate, in consulting the sequential list of codes, the code you have identified from step 3 above.

6. Take note of the branch address opposite the code you have just located in step 5.

7. Address your enquiries about claiming any balance to the branch address identified from step 6.

8. Repeat steps 1 through 7 for each of the other institutions listed in the Table of Contents.

9. If there is no longer a branch of the institution at the address you have identified, address your enquiries to the bank, trust company or loan company as listed below:
